Person-Centred Planning
At Day Services we do not consider person-centred planning (PCP) to be a new technique. PCP is a process of continual listening and learning. Our aim is to focus on what is important to each resident now, and for their future, and to act upon this in alliance with their family, friends and everyone who is significant in their lives.

There are four main approaches to person centred planning.
- Essential Lifestyle Planning
- Planning Alternative Tomorrows with Hope (PATH)
- Maps
- Personal Futures Planning
Each approach is based on the same principles but differs in the way that information is gathered. Emphasis is either on the detail of day-to-day life or on dreaming and more long-term plans for the future, but the end goal is always the same – to share power and encourage community inclusion.
Presently we are rolling out a programme of development around Essential Lifestyle Planning, the preferred planning style although there are trained facilitators able to complete PATHs and MAPs.
All residents presently commence with an Essential lifestyle Plan that focuses on their life now and how it can be improved. This is helpful in ensuring consistency of care, particularly where residents are unable to five their own direction.
