Referrals

Outside admissions to Frederick Harwood are planned well in advance. However, when an emergency admission is required from the David Lewis Centre due to acute health problems, there will be little time for planning or discussion with the multi-disciplinary team. In such cases, admissions are referred by the medical officer, clinical nurse, centre cover nurse or service manager.
Planned referrals are always made to the house’s Consultant and forwarded to our project co-ordinator. The multi-disciplinary team will then discuss the referral at the next team meeting and arrange a home visit.
Two members of the team who we feel are most appropriate to make the initial assessment will make the visit. They will then compile a home visit report, including any recommendations, and forward it to the project co-ordinator who will make an application for funding to your local health authority. Upon receipt of written confirmation of funding, the team will set a date for admission.
