Our Pathways and Community team work in partnership with the families and the carers of everyone we support. This is particularly important during times of transition, where we strive to make sure that each step is carefully planned and people are kept informed.
Family members are invited to formal reviews and informal contact is positively encouraged. We help relatives and carers to participate in the person centred planning process.
We have a family support group which meets quarterly, providing our family members an open forum for discussion and valuable support when they need it. We also have the facilties to provide accommodation for families if required in an emergency or when visiting for the weekend.
